Provider Performance Summary 2025

Overview

The Provider Performance Summary Dashboard is designed exclusively for Medical Directors. It offers an at-a-glance view of the performance metrics for the providers you directly oversee. With an emphasis on accountability, this dashboard aggregates key performance indicators (KPIs) and metrics across the following categories:

  • Patient Experience

  • Operations

  • Clinical Performance

  • Population Health

The Provider Performance Summary dashboard is available in the 'Value Based Care' section of the Insights Portal module.

Best Practices for Medical Directors

  • Regular Monitoring: Review the dashboard frequently to stay informed about team performance and address any emerging issues promptly.

  • Data-Driven Interventions: Use the detailed metrics and drill-down reports to develop targeted coaching and quality improvement initiatives.

  • Team Discussions: Leverage the dashboard during team meetings to highlight successes, discuss challenges, and set performance goals.

  • Benchmarking: Compare your providers’ performance against established benchmarks and industry standards to drive continuous improvement.

Performance Summary Metrics:

The performance summary metrics are:

Total Available Score: Is the total number of points that a provider can earn based on the performance metrics that apply to their patient population. It only includes metrics where there are qualifying patients to evaluate the provider’s performance.

Total Weighted Score: Represents the sum of all points a provider has earned across applicable metrics, with each metric adjusted based on its assigned importance or weight.

Performance Rate: Is the percentage of points a provider has earned out of the Max Available Score, allowing for comparisons between providers with different maximums.

Tabs Overview

Summary by Provider

This tab provides Medical Directors with a clear overview of overall team performance. Medical Directors can choose between a comprehensive table displaying all providers or a category-specific table showing only the providers and metrics relevant to a selected category. Additionally, filters are available to help Medical Directors narrow down the data they want to view.

Tables are broken down by category and include:

  • All Providers

  • Primary Care

  • Virtual Care and Virtual Care On-Demand

  • Urgent Care

  • Home Care

  • Pediatrics Care

  • Medicare

Tables show the Max Available Score, Total Weighted Score, and Performance Rate for each provider.

Summary by Facility

This tab displays the overall performance of each Medical Director, calculated as the average performance rate of the providers they oversee. Providers are grouped according to their primary facility, and each Medical Director's performance is based on the facilities under their management. Tables are broken down by :

  • Regional Director

  • Medical Director

  • Region and County

  • Facility

Data Audit

This tab provides a clear overview of key data elements—such as appointment targets and clinical audits—to help Medical Directors identify any missing or inaccurate information. Tables are broken town by the following categories:

  • Productivity Targets

  • Clinical Audits - Primary Care, Medicare, Pediatrics, and Home Care

  • Clinical Audits - Urgent Care

  • Clinical Audits - Virtual Care and Virtual Care On-Demand

  • Providers by Facility

User Adoption

This tab displays a summary of total and monthly views for Regional Medical Directors, Medical Directors, and Providers, offering insight into how frequently the scorecard is being accessed. Tables are broken day by:

  • Views by Regional Medical Director and Month

  • Views by Medical Director and Month

  • Views by Provider and Month
