Provider Wellness Survey

Overview

Burnout and its contributing factors evolve continuously within healthcare organizations. To address burnout at an organizational level, it first needs to be measured. Sanitas clinicians are surveyed annually to assess their overall well-being, which directly impacts provider retention, engagement, and patient experience.

Clinicians eligible to participate (MD, DO, APRN, PA) must be full-time employees with over 90 days of tenure at the start of the survey and not undergoing resignation or termination.

Confidentiality & Reporting

All individual responses are strictly confidential and anonymized. The data is reported in aggregate to provide insights into overall trends and actionable insights.

Scoring Methodology

The clinician survey uses a 5-point Likert scale for most questions, where clinicians rate their agreement or satisfaction with specific statements:

  • 1: Strongly Disagree

  • 2: Disagree

  • 3: Neutral

  • 4: Agree

  • 5: Strongly Agree

These scores are analyzed to provide both a broad view of overall clinician sentiment and more focused insights into specific areas, such as burnout, support, and workload.

We used two key scoring methods to evaluate burnout drivers, alongside multi-select and freeform questions, which provide a combination of quantitative data and qualitative insights. Together, these methods offer a comprehensive view of clinician experiences.

MiniZ 2.0

The MiniZ 2.0 survey is a validated, concise tool designed to measure the work environment for medical providers. It focuses on key drivers of clinician well-being, including supportive work environments, workload, pace, and electronic medical record (EMR) stress.

MiniZ 2.0 consisted of the first 10 questions in the survey. Each response has an associated numeric value, and a provider’s total MiniZ score is the sum of these values. The overall organizational score is calculated as the median of all clinician scores. A higher score (ranging from 0 to 60) reflects a more favorable work environment, leading to better clinician retention and engagement.

MiniZ 2.0 Subscale Scores

The MiniZ 2.0 also generates two key subscale scores, each ranging from 0 to 30, to provide further detail on specific work environment aspects:

  1. Supportive Work Environment: Measures the degree of support clinicians feel from colleagues, leadership, and the organization. Higher scores indicate a more positive environment that helps reduce stress and increase satisfaction.

  2. Reasonable Pace & EMR Stress: Assesses clinician workload and stress related to the pace of work and EMR use. Lower scores in this area suggest high workload or EMR frustration, both of which are common drivers of burnout.

These subscale scores highlight specific areas for improvement to enhance satisfaction and reduce burnout.

Top 2 Method and Burnout Measures

For survey questions using the 5-point Likert scale, we applied the Top 2 Method to focus on positive responses ("Agree" and "Strongly Agree"). The percentage of respondents selecting these top two options indicates the proportion of clinicians with favorable opinions.

We then aggregated these responses to create an index of burnout drivers, including emotional exhaustion, autonomy, and job satisfaction. This index offers a comprehensive view of clinician well-being across different dimensions.

Multi-Select Questions

Multi-select questions allow clinicians to choose multiple options from a list, capturing a broad range of experiences. For example, clinicians were asked, "What do you like most about working at Sanitas?" with options like "Being part of a team" and "Work-life balance." The results are analyzed by counting the number of selections for each option, revealing the most commonly cited factors.

Freeform Response Questions

Freeform, or open-ended, questions provided clinicians with the opportunity to offer detailed feedback in their own words. These responses are analyzed through thematic analysis, where similar feedback is grouped into common themes. For instance, if multiple clinicians mention issues like "poor scheduling" or "lack of administrative support," these would be identified as recurring themes and used to guide organizational improvements.
