The Consumer Lookup dashboard is a centralized tool designed to provide comprehensive insights into consumer records by using the Lucerna ID (Lucid). By aggregating data from multiple sources, this dashboard enables users to quickly understand and verify consumer information across several key areas.

Metric Overview

The dashboard allows users to search for a consumer using their unique Lucerna ID (Lucid) and retrieves detailed information including:

• Golden Record Demographics: Core personal and contact details.

• Payer Roster: Enrollment data and payer relationships.

• EMR Appointments: History and upcoming appointments from the Electronic Medical Record system.

• Payer Insurance: Insurance plan details and coverage information.

• Consumer Communications: Logs of interactions, messages, and outreach efforts.

• Consumer Surveys: Feedback and satisfaction data from consumer surveys.

This consolidated view facilitates a complete understanding of each consumer’s profile, aiding in record validation and proactive decision-making.

Provides a listed view of consumer details by:

• Name

• Address

• Valid Phone Number

• Email

Roster Details

Contains two tables of roster information sorted by name in one table and consolidated by Luc_ID in the other table.

Patient Insurance Details

Contains patient insurance details and shown by Luc_ID

Appointment Summary

Contains patient appointment summary data such as appointment date and facility, broken down by Luc_ID

Offer History Summary

Contains offer history summary data by Luc_ID and shows Offer Date, Value Driver, Use Case, and Campaign.

Survey Summary

Contains summary survey information by Luc_ID per Visit Date and Provider.
