Attribution and Panel Capacity
Overview
This dashboard delivers a clear summary of the Panel Attribution process outcomes, allowing users to explore results at the provider, facility, or overall summary level. Key metrics provide an at-a-glance understanding of the panel’s profile. Interactive controls let users filter results to focus on the geographies they oversee and adjust the visit timeframe—referred to as the "Seen Rate Basis"—with customizable options of 12, 18, or 24 months.
Metrics Definitions
Panels - Defined in Provider 360, Panels utilize cohorts and/or consumer registries for inclusion and exclusion criteria. There are 3 panels in use today: Adult, Pediatric and Senior.
Base Capacity - The ideal number of patients each provider is targeted to manage. This is set within the Provider Directory’s Provider/Practice Detail page in Leap and is assigned at the Facility/Panel level.
Seen by Any Provider - The count of patients within a provider’s panel who have visited any Primary Care Physician (PCP) during a set timeframe, referred to as the "Seen Rate Basis." This timeframe can be customized by the user, with options of 12, 18, or 24 months.
Base-to-Seen Ratio: Any Provider - The percentage of a provider’s Base Capacity represented by patients who have been seen by any provider, calculated as Seen by Any Provider divided by Base Capacity.
Available Base Capacity - The remaining capacity a provider has, calculated as the difference between their Base Capacity and the number of patients Seen by Any Provider within the specified timeframe.
Seen by Care Team - The count of patients within a provider’s panel who have visited a Primary Care Physician (PCP) within the provider's Care Team during a set timeframe, referred to as the "Seen Rate Basis." This timeframe can be customized by the user, with options of 12, 18, or 24 months.
Base to Seen Ratio: Care Team - The percentage of a provider’s Base Capacity represented by patients who have been seen by any provider within the care team, calculated as Seen by Care Team divided by Base Capacity.
Seen by Assigned Provider - The count of patients within a provider’s panel who have visited their assigned Primary Care Physician (PCP) during a set timeframe, referred to as the "Seen Rate Basis." This timeframe can be customized by the user, with options of 12, 18, or 24 months.
Base to Seen Ratio: Assigned Provider - The percentage of a provider’s Base Capacity represented by patients who have been seen by their assigned provider, calculated as Seen by Assigned Provider divided by Base Capacity.
Flex Capacity - The percentage of flexibility built into each provider’s Base Capacity. Recognizing that not all patients require annual visits, Flex Capacity allows providers to maintain a panel size sufficient to meet their productivity targets. This is set within the Provider Directory’s Provider/Practice Detail page in Leap and is assigned at the Facility/Panel level.
Max Capacity - The upper limit of patients a provider can handle, determined by adding the Base Capacity to the result of multiplying Base Capacity by the Flex Capacity percentage.
Total Assigned: Seen + Not Seen - The complete count of patients assigned to a provider’s panel, encompassing both those who have visited a Primary Care Physician (PCP) and those who have not.
Available Max Capacity - The remaining capacity a provider has, calculated as the difference between their Max Capacity and their Total Assigned: Seen + Not Seen patients.
