Metric Definitions

  • Care Continuity - A metric that measures how effectively a Primary Care Physician’s (PCP) office ensures patients are seen by their assigned PCP or a member of the PCP’s care team, reflecting the consistency of care delivery.

  • Appointment with Assigned Provider - Tracks the percentage of appointments within a specified timeframe that patients completed with their designated Primary Care Physician (PCP), highlighting adherence to assigned care relationships.

  • Appointment with Care Team - Tracks the percentage of appointments within a specified timeframe that patients completed within their designated Primary Care Physician's (PCP) care team, highlighting adherence to assigned care relationships.

Scheduling Team Details

Scheduling Team - Delivers performance metrics for the teams responsible for scheduling patient appointments, featuring drill-down capabilities to the employee’s location and individual names, empowering managers to pinpoint training opportunities.

Scheduling Detail - Offers users the ability to export raw data at the encounter_id level, detailing whether each appointment was scheduled with the appropriate provider, care team, or virtual provider for in-depth review.
